Program Coordinator - Ready Drive (Winchendon)

The program is located in a quiet neighborhood in Winchendon and provides a Group Living Environment (GLE) and community residence to 8 men with mental health diagnoses. The goal is to assist the individuals served with developing the skills needed for independent living. This program offers 24/7 staff coverage. The first floor has handicap access to rooms and bathrooms. Individuals in this program also have support from the integrated team. Services focus on helping people to transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals.
ResponsibilitiesHire, supervise, mentor and evaluate Counselors, and Overnight Staff.Participate in the development and implementation of component goals, objectives, and work plan tasks that are directed toward accomplishing the Agency's Mission and Vision.Facilitate weekly staff meeting.Develop and manage program budget.Ensure optimum program coverage through the development and monitoring of the schedule.Develop program structures and routines which promote a therapeutic milieu which fosters growth and development for all.Responsible for collaborating with Licensed Clinicians and other Integrated Team members to complete all Assessments, Treatment Plans, Service Notes and Periodic Reviews according to prescribed timelines.Responsible for the implementation of high quality rehabilitation according to each person's Treatment Plan.Cultivate mutually respectful partnerships, set appropriate limits, and maintain a helping role with people served and family members.Demonstrate respect for all individuals regardless of ability status, culture, sexual orientation, gender identity, religion, or ethnicity.Assist in the collection, analysis and timely reporting of all rehabilitation billing.Responsible for accurate and timely Events Reporting to the networks' Administrative Assistant.Collaborate with Individuals Served, Licensed Clinician, Registered Nurse, and other members of the team in the implementation and monitoring of any self management and health and wellness plans and activities.Utilize crisis intervention or other means to intervene during a crisis situation, following established program and Agency procedures.Facilitate and monitor the implementation of activities that create opportunities for the development of valued roles and personal relationships in the community.Develop and monitor the implementation of all procedures that ensure the health, safety, and well being of individuals served.Responsible for the correct implementation of all prescribed medical and medication protocols in accordance with agency and MAP policies.Participate in the Quality Improvement process including conducting Surveys and developing/implementing corrective action plans.Assist people to access, and/or provide, all needed transportation.Participate in supervision, team, and other assigned meetings.Review and complete all required documentation.Communicate all pertinent information to all stakeholders.Attend and participate in all internal and external trainings as assigned.Acquire and maintain all required certifications.Participate in On-call Beeper rotation.Perform other duties as assigned.Qualifications
Bachelor's degree in Social Services with residential and administrative experience preferred. High School diploma and valid driver's license required.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
